Sinstralis of the Pain was a demonic gladiator, along with two other demons, Destroyer Zardikar and Talnivarr the Sleeper. She was accompanied by her army of loyal voidwalkers. The trio resided in the Outland Arena waiting for challengers.
 
Sinstralis of the Pain served as a beautiful, seductive, and deadly general in Kil'jaeden's regime. She was one of his many agents on Draenor who delighted in torturing victims for information. After the cataclysm, she found herself stranded on a stray chunk of the planet blasted off by the explosion into the Great Dark Beyond, alone save for two other demons, Destroyer Zardikar and Talnivarr the Sleeper, and her loyal servants.
 
After several years, they held mock wars with each other and their minions. Hers were her spies from Draenor, an army of loyal voidwalkers. Now and then, some adventurer would stumble into the realm and challenge the three demon champions. Always eager to break the boredom, Sinstralis would engage them with little provocation.
 
After the third war the beastmaster Rexxar, Cairne Bloodhoof, Rokhan and Chen Stormstout came across a portal to the arena while traveling through the Barrens. They fought Sinstralis of the Pain in order to gain access to her library of agility tomes.
